The Blood Lance Necromancer delivers powerful single-target damage, aiming to melt bosses while using Blood Orbs and the "Lanced" mechanic to create AoE attacks that wipe out entire screens. This build reaches its peak damage through guaranteed Overpower abilities from Rathma's Vigor.

The "Lanced" mechanic introduces a unique playstyle in Diablo 4. Each ‍Blood Lance remains in an enemy for 3 seconds, and additional ‍Blood Lance not only hit the original target but also deal damage to all enemies already Lanced.

This build focuses on stacking Critical Strike Chance and Damage to boost your Overpower hits. When your primary Lance lands a critical hit, all Lanced targets receive that bonus damage. With the ability to apply Vulnerable, the Blood Lance Necromancer can tap into every damage multiplier. Add on the ability to stack a ton of Maximum Life and the Fortify status, this build is a natural Tank and easy to survive on Hardcore.

  • Mutilator Plate increases the effectiveness of your build multiple times over. It scales your damage, helps to reach high % Blood Lance Deals Double Damage, and generates Blood Orbs.

The Season 12 theme brings Killstreaks and Bloodied Items to Diablo 4. Slay enemies in succession to begin a streak that grants increasingly greater benefits based on which affixes you roll on your gear in addition to seasonal reputation and experience. Tiers are divided as follows: Killstreak (0-99) - Carnage (100-249) - Devastation (250-499) - Bloodbath (500-999) - Massacre (1000-9000) - Fresh Meat! (9001+). For most instanced content types, you can typically expect to reach up to the 4th tier (Bloodbath). Additionally, you can find Bloodied Sigils for Nightmare Dungeons, Infernal Hordes and Lair Bosses that offer greater challenges and rewards (introducing "pseudo Torment 5").

Bloodied Items

Bloodied affixes are bonus affixes that can randomly roll on gear and come in three categories: Rampage (armor), Feast (weapons), Hunger (jewelry). The first two offer minor combat effects while the latter boosts specific loot drops, which doesn't impact your build. Recommended affixes for this build are:

  • Rampage: Increased Main Stat, Gluttony(Max life), Heartbeat (Lucky Hit Chance), Keen(Attack Speed)
  • Feast: Tenderize (Berserking)
  • Hunger: Increased Chance for Runes, Bloodied Items, Increased Experience

Gameplay Notes
  • Reap is used to get resources back quickly. It also boosts damage and movement speed via Reaper's Pursuit.
  • Blood Lance is the main damage dealer. The "lanced" mechanic works both as a single target and as an AoE. Blood Lance lingers on enemies for 3 seconds, and makes the target “lanced”. Whenever a Blood Lance deals damage, all lanced targets also take that damage. Grouping enemies with Corpse Tendrils helps significantly when it comes to lancing multiple targets quickly.
  • Corpse Tendrils groups enemies together and generates Blood Orbs. Blood Orbs are the engine of Blood Lance Necromancer. They help you Overpower more frequently by activating Rathma's Vigor as well as giving you the essence you need.
  • Decrepify applies a powerful CC effect that also causes Lucky Hit procs to reduce Cooldowns on your Skills. You do not manually cast it, Cursed Aura or the Wat rune does the job for you!
  • Soulrift is used in the early game as a fantastic tool for Resource management and Vulnerable application.
  • Sever and Blight are only on the skill bar to increase damage and movement speed via Reaper's Pursuit in the endgame. The ranks are granted from your multiple sources of "+x to Core Skills". You can replace this with Blood Mist if you want the survivability tool.
Rotation Breakdown

Skill Rotation

  • If Soulrift is off Cooldown, recast it before engaging a pack of enemies.
  • Blood Lance targets from distance while approaching to aggro them. This triggers Hewed Flesh. If it doesn't, cast Reap into the pack to do so.
  • If your essence is fully depleted, also utilize Reap to quickly get some essence back.
  • Once a corpse is available, you cast Corpse Tendrils. It is quite important to cast Corpse Tendrils on cooldown so you activate Grasping Veins to boost critical strike chance as well as stunning enemies and generating Blood Orbs.
  • If you have Tam, continue to cast Lances to trigger Decrepify on the pack. If you're using Cursed Aura still, walk into the pack to apply it.
  • Once enemies are prepped, drop a Bone Prison on them to keep them together and apply Vulnerable.
  • The more Blood Orbs you collect by casting blood lance via Gore Quills, the more you activate Rathma's Vigor and Overpower.
  • You continue to spam cast Blood Lance as much as possible and Corpse Tendrils whenever it's off cooldown.
  • If you prefer the safety and Immunity for Hardcore playstyle, replace Sever on your Skillbar with Blood Mist. Cast this whenever to avoid a dangerous situation, or you've been Crowd Controlled to break the CC.
  • If your essence is fully depleted, utilize Reap to quickly get some essence back to be able to continue spamming Blood Lance.


Book of the Dead

The Blood Lance Necromancer chooses to sacrifice Skeletal Warriors - Defenders to increase your resistances especially while pushing high pit or tower tiers, Skeletal Mages - Bone for the massive increase to Overpower Damage and Golems - Iron for Critical Strike Damage bonus.

If you have not reached 100% critical strike chance during the early game, you can sacrifice Skeletal Warriors - Skirmishers for extra crit chance.

  • If you're feeling squishy or playing on Hardcore, sacrificing ‍Golems - Blood for Maximum Life is a great ways to beef up your survivability.

Blood Lance Necromancer

This section is designed to guide your Blood Lance Necromancer from a fresh level 60 into the very late endgame using the three variants Starter, Ancestral and Mythic. At the final step, you should be fully decked out with a great all-rounder build. From there, follow the other min/max variants if you want to optimize around certain activities such as Push (Tower/Pit), Speedfarm and more.

If you are fresh in Torment 1 coming from a leveling guide, this is where you begin. Since only some Aspects can be guaranteed from Dungeon unlocks for the Codex of Power, focus on acquiring the additional aspects via gambling with Murmuring Obols.

  • Early on in your progression, focus on stacking Maximum Life to survive. As you unlock higher power gear and Paragon points, you naturally scale your character into the higher difficulties and can start replacing some of these rolls with offensive stats or uniques.
  • Once you cap resistance and armor, focus on Essence sustain and attack speed in the early game. As you gain more paragon levels and improve your gear, the damage will come.
  • Soulrift Duration temper helps to maintain great uptime on this Skill, especially in the early game.

This section describes how to optimize your Blood Lance Necromancer with the right Item Affixes, Runes and Mercenary choices.

To enable the Blood Lance Necromancer to perform optimally, hunt for these stats on your gear with good rolls and aim to Temper ⚒️ & Masterwork Capstone them accordingly. Keep in mind that certain stats, such as Extra Size Tempers, Attack Speed, Critical Strike Chance and more, have caps that may affect your priorities according to the items and Paragon points available at that moment.

Recommended Consumables: Soothing Spices, Reddamine Buzz for more survivability. Sage's Whisper for Intelligence and Elixir of Advantage II for Attack Speed + Lucky Hit Chance. If you need more survivability, Elixir of Fortitude II is a good option instead of Elixir of Advantage II.

Once a guide is released, many players have questions on why and how we are approaching certain choices. In this section you can find the answers to the questions that have been asked the most.

Frequently Asked Questions

The build works really well when you decimate everything in seconds, but may get overwhelmed if fights are long against high level monsters. There are several adjustments you can make when you have trouble with more difficult content:

  • Focus more on Armor & Resists, making sure that they are always capped especially as you transition into higher torment difficulties.
  • Use defensive consumables. Especially the Elixir of Fortitude II.
  • You can swap Golems - Iron sacrifice with Golems - Blood sacrifice to maximize your tankiness if necessary.
  • Focus more on maximum life rolls, for example replacing DPS stats like main attributes, attack speed, critical strike chance and additive increased damage rolls. Also masterwork life more than normal if no other crucial stats have priority.
  • Focus on adding defensive skills, passives and paragon nodes first as you level up.
  • For hardcore, play on a slightly lower difficulty than normal, don't push into difficult territory. As long as you can deal with all enemies swiftly, they can't retaliate.

Mechanics

  • Blood Lance lingers in a target it hits for 3 seconds. Whenever a new Lance hits another target, ALL lanced targets take the damage dealt by the new Lance.
  • This procs for the multitude of different Lance generating Aspects and effects in your Skill Tree.
  • When an attack successfully Overpowers, that Lance and all other lingering Lances, apply that Overpower damage. The same goes for Critical Strike damage and Vulnerable etc.
  • When a Lance hits a Lanced target, it spreads a Lance to another nearby target, which can deal damage and apply that damage to all currently Lanced targets as well.
