The Fleshrender Druid is defined by the Fleshrender unique, which damages Poisoned enemies when you trigger your Defensive Skills. Its remarkable lethality comes from its unique effect, which scales the damage of your Defensive Skills with Willpower. It should be emphasized that Fleshrender's damage activates on all triggers of Defensive Skills, not just manual casts. This allows Fleshrender to interact with the likes of Nature's Fury and Defensive Runes for considerably more procs.
Out of the available Defensive Skills that can benefit from Fleshrender, Earthen Bulwark stands out due to its attractive Earth and Werebear multipliers when paired with Vasily's Prayer. To maximize the powerhouse that is the Fleshrender Druid, utilize Nature's Fury and Symbiotic to reset and cast Earthen Bulwark as often as possible.
When it comes to cooldowns, this build features two very important cooldown thresholds. Firstly, ensure your Earthen Bulwark's and Cyclone Armor's cooldowns are lower than 6 seconds to instantly reset them using Symbiotic. Secondly, acquire enough Cooldown Reduction for permanent Cataclysm uptime, which is responsible for applying Poison and Vulnerable to enemies.

Fleshrender Druid
The Season 12 theme brings Killstreaks and Bloodied Items to Diablo 4. Slay enemies in succession to begin a streak that grants increasingly greater benefits based on which affixes you roll on your gear in addition to seasonal reputation and experience. Tiers are divided as follows: Killstreak (0-99) - Carnage (100-249) - Devastation (250-499) - Bloodbath (500-999) - Massacre (1000-9000) - Fresh Meat! (9001+). For most instanced content types, you can typically expect to reach up to the 4th tier (Bloodbath). Additionally, you can find Bloodied Sigils for Nightmare Dungeons, Infernal Hordes and Lair Bosses that offer greater challenges and rewards (introducing "pseudo Torment 5").
Bloodied Items
Bloodied affixes are bonus affixes that can randomly roll on gear and come in three categories: Rampage (armor), Feast (weapons), and Hunger (jewelry). The first two offer minor combat effects while the latter boosts specific loot drops, which doesn't impact your build. Recommended affixes for this build are:
- Rampage: Primary Core Stat (especially good for Fleshrender), Attack Speed, Cooldown Reduction, Maximum Life, Critical Strike Chance
- Feast: Berserking, Chains, BloodSplosion
- Hunger: Prioritize increased XP. Otherwise, use what you prefer/have access to.

Gameplay Notes
- Storm Strike is your Basic Skill, serving two purposes. Firstly, you use this skill to trigger Nature's Fury, which resets Earthen Bulwark's Cooldown thanks to Symbiotic. Secondly, you cast it frequently to activate Natural Balance's damage multiplier, increasing your Earthen Bulwark's damage. What's more, Storm Strike provides Damage Reduction and can be used while moving.
- Earthen Bulwark is your main Fleshrender damage dealer. Allocate skill points to Enhanced Earthen Bulwark and Preserving Earthen Bulwark for Unstoppable and additional Fortify generation. It is essential for this skill's Cooldown to be sub 6 seconds to allow Symbiotic to reset it instantly when it activates. Use Vasily's Prayer to enhance this skill into a Werebear Skill, granting you access to Changeling's Debt and the Survival Instincts Legendary Paragon Node.
- Cyclone Armor applies Vulnerable to enemies it hits and features a damage multiplier functionality in Innate Cyclone Armor. Similarly to Earthen Bulwark, ensure its Cooldown is sub 6 seconds.
- Debilitating Roar and Blood Howl offer various defensive and offensive bonuses. These skills grant you increased damage, Damage Reduction, Healing, and Attack Speed. Cast Blood Howl regularly to double Toxic Claws' damage multiplier.
- Cataclysm is your Ultimate Skill of choice, and thanks to Overload, it's fantastic at applying Poison and Vulnerable to enemies, including off-screen targets. Its long-range capability pairs nicely with Fleshrender's wide damage radius, allowing you to delete screens of Poisoned enemies at once. Temper Cataclysm Cooldown Reduction to obtain permanent uptime on this skill.
Rotation Breakdown
Skill Rotation
This gameplay loop requires multiple button inputs. As a result, this build is best enjoyed with a controller.
- When entering a new zone, cast Cataclysm to begin applying Poison and Vulnerable to your enemies.
- Follow up by casting Earthen Bulwark and Cyclone Armor to eliminate your targets. If you're using a controller, you can hold down these two skills and your character will cast them as soon as they're available.
- Repeatedly cast Storm Strike to proc Nature's Fury and Symbiotic, which triggers even more Defensive Skills and resets your Earthen Bulwark and Cyclone Armor cooldowns. It is also recommended to hold down this skill.
- Activate Debilitating Roar and Blood Howl as necessary for bursts of damage, Damage Reduction, Healing, and Attack Speed. Ensure you're casting Blood Howl every 8 seconds to double the bonus from Toxic Claws.
Spirit Boons
Druids can choose 5 out of 16 Passive bonuses (Boons) from 4 different Spirits. Bond with one of the Spirits (by clicking its Portrait in the Spirit Boon menu) to activate two of its Boons. Use the following Boons:
Deer Spirit: Wariness
Eagle Spirit: Avian Wrath
Wolf Spirit: Calamity
Snake Spirit: Overload
Bond with the Eagle Spirit and activate Iron Feather for additional Maximum Life. If you prefer more damage, opt for Swooping Attacks.

Fleshrender Druid
This section is designed to guide your Fleshrender Druid into the very late endgame using the variants Ancestral and Mythic. At the final step, you should be fully decked out with a great all-rounder build. From there, follow the other min/max variants if you want to optimize around certain activities such as Push (Tower/Pit), Speedfarm and more.
The goal here is to progress through the Torment difficulties as you unlock Ancestrals, Paragon points, all Legendary Aspects, Temper mods and the most important Uniques to pilot this build. Some Legendary Aspects are used temporarily to fill slots that may be replaced with more Unique items later on.

Before transitioning to this build, ensure you have a well-rolled Symbiotic aspect with 5 or 6 seconds. While it is possible to play the build without it, this aspect substantially increases the rate at which you can trigger your Defensive Skills for more Fleshrender activations. It is recommended to gamble Obols for Boots to farm this aspect.
- The Fleshrender Druid cannot be played without a Fleshrender. As such, prioritize obtaining this item from Lord Zir, with an emphasis on Ancestral versions with high unique effects. If possible, try to acquire a multi-GA version of this item, as all stats are desirable.
- Khamsin Steppewalkers, a newly introduced item last season, helps you meet the Cooldown Reduction requirements for this build, and provides you with max Movement Speed and Unhindered. Aim to acquire this item early from Grigoire, The Galvanic Saint.
- Mjölnic Ryng offers a better-than-average multiplier for this build, and extends Cataclysm's duration. Grab this unique from Lord Zir.
- Vasily's Prayer converts your Earthen Bulwark into a Werebear Skill, granting you access to Rabid Beast, Changeling's Debt, Survival Instincts and various other Werebear defensive Passives. The %Willpower affix present on this item also substantially scales your Fleshrender. This item is found at Echo of Varshan.
- Tibault's Will offers a healthy amount of All Stats and Attack Speed, and pairs nicely with Enhanced Earthen Bulwark for a solid damage multiplier. This unique is obtainable from Duriel, King of Maggots, and/or Andariel.

Prime Grizzly Rage grants permanent Unstoppable while in Grizzly Rage. Alternatively, Druids have only two non-Ultimate sources of Unstoppable; Trample and Earthen Bulwark. If you're not subscribed to their long Cooldown, Metamorphosis is a great alternative to those skills.
Mechanics
Vasily's Prayer makes your Earth Skills also count as Werebear Skills, Fortifying up to 0.5% of your Maximum Health with every cast. While the Helm offers significant value with its Armor, Life, and %Willpower, the real value lies in its ability to turn Earth Skills into Werebear Skills.
