Welcome to the Death Blow Barbarian Endgame Build Guide! This build revolves around repeatedly casting powerful Death Blow, enabling a fast-paced, generatorless playstyle capable of mowing down even the toughest enemies, such as the Season 12 Butchers, thanks to Enhanced Death Blow!
Death Blow Barbarian Endgame Build Guide for Diablo IV Season 12 - Slaughter
This playstyle is enabled thanks to The Third Blade, which turns Death Blow into a spammable Core Skill with a baseline Fury cost. We then scale that cost to 100+ Fury per cast through Crown of Lucion, allowing every cast to trigger powerful bonuses such as Ring of Red Furor and Encroaching Wrath. Fury sustain is maintained through Resource generation bonuses paired with the Fighter's Death Blow node.
Overkill further enhances the build, allowing Death Blow to launch deadly shockwaves with every cast while also providing excellent stats.
In the late game, the build leverages the "Selig Low Life" combo as it remains the strongest gearing setup of the season (even without Season 11 Sanctifications); meaning we greatly increase our Fury pool to scale the bonuses from Ramaladni's Magnum Opus while keeping our Health as low as possible to minimize the damage redirected by Melted Heart of Selig, effectively allowing us to not care about defenses at all.
Requirements for this build to function:
- The Third Blade

This is the final version of the build's progression, including all regular Unique items, Mythic Uniques, Legendary Aspects, Temper mods and Masterworking, leaving you with a great all-rounder build for all content in the game. Additional variants use this as a baseline to optimize around certain activities.
Gear & Skills
How it Works
Skills & Rotation
- (2H Mace) Death Blow is your primary damage dealer, turned into a spammable Core skill through The Third Blade that both costs and generates Fury thanks to Fighter's Death Blow.
- (2H Mace) Hammer of the Ancients as a Fury pool and Fury generation steroid thanks to the Enhanced Hammer of the Ancients stacks. Violent Hammer of the Ancients also provides a massive damage boost when your cast Overpowers.
- (Dual wield) Lunging Strike as a place-holder skill to trigger our Dual Wield Walking Arsenal bonus, it also doubles up as way to quickly move in and out of packs.
- (Two-Handed Slash) Mighty Throw to trigger the remaining Two-Handed Slash Walking Arsenal bonus and provide us a big Attack Speed steroid through Enhanced Mighty Throw.
- Call of the Ancients gives Massive Damage, Movement and Attack Speed multipliers. The Ancients also generate Fury every attack.
- Ground Stomp as a triple-purpose support powerhouse.
- Spawns Earthquakes and activates their generic multipliers (Earthquakes)
- Reduces the Cooldown of our Ultimate
- Generates Fury and pulls enemies together
- (Auto-cast via Runes) War Cry as our offensive Shout, grants us Berserking and a hefty damage multiplier.
Skill Rotation
Buff Rotation - Whenever available
- Call of the Ancients to generate massive amounts of Fury and benefit from its many buffs
- Ground Stomp to pull in monsters
Buff Rotation - Once every 4-5seconds
- Lunging Strike to activate the Dual-Wield Walking Arsenal bonus
- Hammer of the Ancients to gain or refresh your Enhanced Hammer of the Ancients stacks (up to 10).
- DISCLAIMER: For optimal damage output and Fury sustain, keeping your 10 stacks is very important.
- Mighty Throw to activate the Two-Handed Slash Walking Arsenal and Enhanced Mighty Throw bonuses.
Main Rotation
- Spam Death Blow in the general direction of your foes.
Gear
- The Third Blade - Requirement to make Death Blow a spammable Core skill with a Fury Cost.
- Crown of Lucion - Highest damage multiplier (x105% with the S12 changes) and increases the cost of our Skills, which we absolutely want to unlock our synergies.
- Overkill - Massive stats, bonus ranks and a multiplier on top of the ranged shockwave effect.
- Ring of Red Furor - Perfect offensive stats on top of a massive damage multiplier and guaranteed critical strikes every cast.
- Ramaladni's Magnum Opus - Unique effect with a massive Fury based damage multiplier that works as long as the weapon is equipped.
- Tibault's Will - Additional Resource, offensive affixes and a decent damage multiplier on top.
- Melted Heart of Selig - Massive stats increase, doubles your Fury and makes you immortal as long you remove all Health bonuses from your gear/paragon/consumables.
- Shroud of False Death - Multitude of small benefits that add up to a ~30-50% damage gain.
